The clinical examination module sets the pace. A patient arrives with non-specific chest pain. You select an examination type, watch the doctor palpate, then decide: ECG and blood panel, or send them straight to a specialist. One wrong referral clogs the queue.

Run the diagnostic lab

Order laboratory tests and imaging. Slide a CT patient into the scanner, draw blood, wait for results. Each piece of equipment occupies a room you’ve built, so the lab’s layout directly affects turnaround time.

Manage surgery and hospitalisation

Admit critical cases and schedule operations. The surgical team moves through pre-op, theatre, and recovery. You track bed occupancy and decide when a stabilised patient can transfer out of ICU.

Build the next floor

Switch to multi-floor construction mode. Place prebuilt elements—waiting areas, nurses’ stations, on-call rooms—and rearrange corridors to reduce walking distances. A preset layout gets you started, but tweaking the floor plan is where patient flow improves.

Break the waiting-room loop with a room reassign

Patients looping in the waiting room. If a transferred patient bounces between reception and a specialist door without being seen, pause the game. Reassign the specialist to a different room or briefly close the department. Unpausing usually forces the patient record to recalculate the next available slot.

Game fails to load on launch. Mac systems sometimes sprinkle hidden AppleDouble files into the game directory. The 1.2.23726 update fixed that, but if you still hit a black screen, check for ._ prefix files inside the ProjectHospital.app bundle and remove them via Terminal. Ambulance patients expiring before diagnosis. Keep a dedicated triage nurse on the ER desk at all times. If the default shift rota leaves a gap, the queue stalls and urgent cases deteriorate. Set a second nurse to cover the handover between day and night shifts.

Orthopaedic referrals blocked from ICU. Before the latest patch, a patient needing a Boston brace couldn’t move from intensive care to orthopaedics. The fix is live now. If an older save still shows the block, discharge the patient and readmit them through the emergency entrance.

Drop diagnosis difficulty to save early reputation

• Diagnosis difficulty. Drop it to the middle setting for the first hospital. Full realism sends symptom descriptions you’ll misread early on, and every misdiagnosis costs reputation you need for insurance contracts.
• Auto-save interval. Default is 15 minutes. Bump it to 5 while you learn the construction tools — one accidental wall deletion can strand a department.
• Staff autonomy. Switch patient assignment from manual to semi-automatic under the gameplay tab. Doctors will still wait for your diagnostic orders, but they’ll handle routine ward rounds without your clicking every bed.
• Insurance contract view. Open the contracts screen and pin it to the HUD. You need to see which insurers cover which conditions before accepting a walk-in, or you’ll treat uninsured patients for free.
